I'd say it's just as likely a result of a general lack of respect secondary to Obama's age and what the right considered to be a "light resume." It definitely showed disrespect and was completely uncalled for.
As a Democrat, I take solace in the fact that the Republicans have nominated the guy who lost the nomination to the guy who lost the presidency to the guy who is now the President.

Perhaps Joe Wilson simply yelled "You Lie!" because Obama insulted his intelligence and flat out lied to his face.
